Alopecia Areata

The most common type of Alopecia Areata presents as round or oval patches of hair loss, most obvious on the scalp or in the eybrows. Those who develop round or oval areas of hair loss can progress to toatal scalp hair loss. (Alopecia Totalis). Traction Alopecia

Trichotillomania

Trichotillomania is a traction related form of alopecia. Trichotillomania is the name given to the habitual and compulsive plucking of hair from the scalp and/or other hair-bearing areas of the body. Long-term trichotillomania can result in permanent irrevocable damage. It remains unknown whether trichotillomania should be classified as a habit or as obsessive-compulsive behavior. In its mildest form, trichotillomania is a habitual plucking of hair while a person reads or watches television. In its more severe forms, trichotillomaniahas a ritualistic pattern and the hair-plucking may be conducted in front of a mirror. The person with trichotillomania often has guilt feelings about his or her “odd” behavior and will attempt to conceal it. Hair loss due to trichotillomaniais typically patchy, as compulsive hair pullers tend to concentrate the pulling in selected areas. Scarring Alopecia

Hair loss due to scarring of the scalp is called scarring alopecia. Scarring can be due to a variety of causes. Traction alopecia over a period of time may lead to scarring and permanent hair loss. Similarly, trichotillomania(compulsive hair-plucking) can cause permanent scalp scarring over time. Injury to the scalp caused by physical trauma or burns may leave permanent scars and permanent hair loss. Diseases that may cause permanent hair loss due to scalp scarring include (1) the autoimmune conditions lupus erythematosusand scleroderma, and (2) bacterial infections such as folliculitis, fungal infections, and viral infections such as shingles (Herpes Zoster). Triangular Alopecia

Triangular alopecia is loss of hair in the temporal areas that sometimes begins in childhood. Hair loss may be complete, or a few fine, thin diameter hairs may remain.   • At the end of the Anagen phase
  • It lasts about one or two weeks

The Telogen Phase - Resting phase

  • It follows the Catagen phase and normally lasts about 5-6 weeks
  • At any given instance in time, approximately 10-15% of all hair follicles are in this phase
  • The hair does not grow but stays attached to the follicle
  • The dermal papilla stays in a resting phase below

The production of hair is not continuous but cyclic. The growth phase (Anagen) is succeeded through a transitional phase of regression (Catagen), by the rest phase (Telogen), and the cycle is preserved.During the relatively long Anagen phase (lasts for 1,000 days, i.e. 2-5 years), the matrix cells divide, become keratinized and hair is produced. The Catagen phase lasts for about 2-3 weeks and is practically unimportant. However, women with hair loss due to this cause usually do not develop true baldness in the patterns that occur in men; women rarely develop the “cueball” appearance often seen in male-pattern androgenetic alopecia.  Patterns of female androgenetic alopecia can vary considerably in appearance.  

Patterns of Female Hair Loss

Patterns that may occur include:
  • Diffuse thinning of hair over the entire scalp, often with more noticeable thinning toward the back of the scalp.
  • Diffuse thinning over the entire scalp, with more noticeable thinning toward the front of the scalp but not involving the hairline.
  • Diffuse thinning over the entire scalp, with more noticeable thinning toward the front of the scalp, involving and sometimes the breaching the hairline
Unlike the case for men, thinning scalp hair in women due to androgenetic alopecia does not uniformly grow smaller in diameter (miniaturise). Women with hair loss due to androgenetic alopecia tend to have miniaturising hairs of variable diameter throughout the affected areas of the scalp. While miniaturising hairs are a feature of androgenetic alopecia, miniaturisation may also be associated with other causes and is not in itself a diagnostic feature of androgenetic alopecia. In post-menopausal women, for example, hair may begin to miniaturise and become difficult to style. The precise diagnosis should be made by a physician hair restoration specialist

    Would you have a 'lunch-hour' hair transplant? What happened when one man had the £7,000 procedure

    I never thought I’d be the type of man to have a hair transplant.

    Sure, the likes of James Nesbitt, Robbie Williams and Louis Walsh have gone under the knife to bolster their barnets, but I always thought hair loss should be handled with a little less fuss - and a lot more stoicism.

    Not least because, at 34 years old, I still had (what could truthfully be described as) a full mane – certainly no bald spots, just a very slight receding hair line.

    And that’s official. According to the Norwood scale, the industry-wide guide which measures the stages of male pattern hair loss, I was only just in category two, which is the very first rung of the ladder.

    Thankfully, nature had not intended for me to look like a billiard ball.

    But, when on a journalist commission to find the hot new trends in male cosmetic surgery, I stumbled across a procedure that has since been dubbed the ‘lunch hour’ hair transplant.

    This is because of its discreetness and simplicity: DHI or, to give it its full name, direct hair implantation.

    Too good to be true? It certainly seemed so.

    To test this theory I posed as a keen customer during consultations with several different companies, generating vastly different results. 

    One Harley Street doctor quoted me £17,000 and said I'd need several high-cost operations.

    Another, based in mainland Europe, offered his services for ‘just’ £10,000, but the price of back-and-forth international travel wasn’t included. 

    Then, a more local practitioner suggested I undergo the old-fashioned ‘strip’ transplant, which involves cutting large chunks of the scalp away and replanting them.

    A procedure which isn't just fantastically painful, but also unsightly: leaving a huge, tell-tale horizontal scars at the back of the head.

    Hardly ideal if I wanted something low-key, which is surely the watchword of any procedure. Especially for men.

    But then, through my love for sport, I found DHI Medical Group – who, over 40 years, have worked with the likes of cricketer Phil Tufnell and Dutch International football Ronald de Boer. 

    Having met both of them on separate occasions, they were both enthusiastic about their experience and, more importantly, had impressive hairlines to prove it.

    So I delved a little deeper – and booked in with a consultant at their New Cavendish Street branch in London, who not only detailed what would happen during the seven-hour procedure, but also guaranteed lifetime hair growth and 100 per cent safety.

    This, they assured me, would be achieved by extracting hair follicles individually from the donor area (the lower back of the head that’s rarely affected by baldness), using a specifically-designed tool with a diameter of less than 1mm to avoid scarring.

    Then, the follicles would be implanted one-by-one directly into the region suffering hair loss, creating cumulative density. No surgery, no stitches, no overnight stay.

    It seemed too good to be true, but within days I was booked in.

    My transformation started with a military style haircut.

'Hardly scary stuff,' he said[/vc_column_text][/vc_column_inner][vc_column_inner width="1/2"][vc_single_image image="29653" border_color="grey" img_link_large="yes" img_link_target="_self" image_hovers="true" img_size="large"][vc_column_text]The hair follicles are kept at a temperature and in a solution that enhances their development, the clinic says [/vc_column_text][/vc_column_inner][/vc_row_inner][dt_gap height="10"][vc_column_text]

    Historically, men have been forced to shave their entire heads in order for doctors to collect fresh roots from growth areas.

    But my surgeon offered me the option of a fashionable 'blend' cut, which - at worst - looked like I should be in Shoreditch.

    Then, it was into the clinic room - which is where the magic happens. Here, they used local anaesthetic to numb the spot where they harvested healthy hairs to be replanted. 

    This itself was surprisingly painless. 

    In fact, the worst part was probably having to lay face-down on a surgical table (with my face peeping through one of those embarrassing holes) for so long.

    But the procedure - which collected around 10,000 grafts - was gentle and precise, with minimal fuss. Painstaking for the surgeon, yes, but pain-free for me.

    A quick lunch break, then it was part two: the implantation.

    The border of my original hairline was marked out with a marker pen - leaving a surface area to be filled with the plucked hair. 

    Flipping me over onto my front, it continued as we had started: while chatting and listened to the radio. Hardly scary stuff.

    Intrigued, I watched in a hand-held mirror as my younger hairline suddenly re-appeared across my face. 

    And, although I hadn't stayed awake at night grieving it, I was delighted. 

    The sensation of the hairs being re-planted felt a bit like being prodded in the scalp with a biro, which was a small price to pay for reversing years of cruel shedding.

    Why had more people not heard of this?

    From this point, with the initial surgery done, I was a new (albeit cautious) man. 

    Doctors stressed that the first few weeks post-transplant required the greatest care, both in how I kept my scalp clean, but also in ensuring I didn't damage the grafts.

    They briefed me on the horror stories of men who'd spent hours having the procedure done - only to bang their head getting into the taxi home, leaving clumps of transplanted hair strands in their hands.

    This meant being hyper-vigilant about how I spent my time during the day and night, avoiding sun exposure in the former and sleeping with a head brace to stop potential pillow friction. Showering also had to be done with caution.

    Within a fortnight I experienced the unsightly scabbing period, which - don't panic – isn’t a good look, but is simply the new, transplanted hair being accepted into the scalp.

    And, once this cleared up with regular washing, it was all plain-sailing.

    Today, one year on, I’m delighted – and pretty amazed.

    It's only now, when I look back at old photographs of myself, that I realise what a difference it has made. 

    Increasingly, the hair has not only grown longer, but thicker too: assimilating almost perfectly.

    Granted, I didn't necessarily need the procedure doing in the first place – yes, there are men and women who have much more extreme cases of hair loss - but the ease of the procedure made it both practical and safe, not to mention convenient. 

    So why not? 

    Now, I forget I even had the work done because the results are so natural - and, likewise, people I meet are none the wiser. 

    In fact, many of them make reference to my luck of having such good hair genes. 

    I feel like a new man.

As per the survey, DHI received the highest rating of 8.3 for overall client satisfaction, indicating highly effective services that met customers’ expectations. India has witnessed a significant boom in the hair transplant industry in recent times, leading to a proliferation of hair restoration centers. The hair transplant industry is highly fragmented and consists of a large number of independent doctor practitioners. Some of the key players are DHI, Advanced Hair Studio, Rich Feel, Berkowits , ADHI, Dr Batra and Reviva. Given the landscape of the hair restoration industry in India, IMRB in association with RedSeer Consulting conducted a survey among 280 clients of various hair restoration clinics across Bangalore, Delhi, Mumbai, Chennai, Kolkata and other tier 1 cities. The survey was conducted on a range of parameters to analyze the actual experience of the consumers, who have gone through the treatment at major hair restoration clinics in the country. The survey reveals that -
    • On consultation DHI was far ahead of competition scoring 7.37 on the scale of 10 and the patients were content with the consultation as the treatment was customized based on their condition and was not a “universal fit” or based on marketing-led initiatives
    • DHI had a high score of 8.8 out of 10 for the level of professionalism, qualification, experience and expertise of the DHI team at the clinic. DHI did well on reliability in providing pre and post procedure information, expected results, and temporary after effects of hair transplantation - setting the right expectations for the customers.
    • Transparency in pricing is the key to earn customer trust and influences the decision on undertaking the procedure. The patients feel that DHI was performing detailed diagnosis of the alopecia condition and DHI received a perfect score of 100% for the transparency that the clinic has in the cost of the procedure.
    • Level of pain experienced during the treatment is among one of the important considerations to determine customer satisfaction levels, and the patients at DHI rated the clinic at 8.4 describing it as a painless experience.
    • Adding to the treatment, patient care and clinic infrastructure enhances the entire experience of the procedure and DHI had an average rating of 8.7, while most other clinics did not score more than 3.7 on infrastructure and staff behaviour
    • The high level of DHI customer satisfaction is reflected in the fact that most patients are extremely likely to recommend DHI treatment to others.
    • Among the parameters affecting the decisions to go for hair restoration - availability of best in class technology is considered the most important. Referrals by doctors and reputation of the clinic are also key. Only 12% of the respondents were influenced by the Ads they saw. Most said they did not consider the cost of treatment when making the decision. (Ref Figure 1)

  • Men who actively keep fit may be inadvertently speeding-up hair loss. According to experts, weight-lifting in the gym may play a key role in a man’s receding mane – especially if combined with the consumption of popular protein powders. The theory is based on the belief that, when combined, these two factors increase the body’s testosterone and Dihydrotestosterone (DHT) levels, which are considered key causes of baldness. Although naturally-produced, DHT – which is a converted version of testosterone – quickly attaches itself to a man’s hair strand’s receptor cells, blocking the absorption of essential nutrients. Over time, this gradually causes the follicles to shrink and the hair to wither until it eventually dies. Thus, by fitness fanatics increasing their exposure to DHT, gym-goers are also risking an earlier on-set of shedding and loss. Currently, there are a relatively limited treatments available. Clinical trials repeatedly prove that the only real options are Minoxidil and Finasteride. At the moment, over 90 per cent of hair loss treatments contain the compound Minoxidil, which was originally created as a blood thinning agent but works in some men to reduce hair loss. It can be effective if used early to prevent hair loss rather than to grow new hair and is found in the UK in products such as Regaine. Meanwhile, Finasteride – an oral tablet – reduces DHT by blocking 5-alpha reductase. It was not originally designed to combat baldness, but began to appear in pharmacies when some men being treated for enlarged prostates – also thought to be caused by increased levels of DHT – noticed their hair had not only stopped receding but had begun to grow back thicker and stronger than before.     Another Academy Masters Meeting has come to the end. This one was probably one of the best ever organized by London Hair Restoration Training Academy. Once again, DHI Global Medical Group proved that it is by far the largest and most experienced medical team of the world in hair restoration. With our new innovative research projects, we have taken things to the highest level of scientific research and this is only the beginning.

    Day One: Saturday, May 28th

    Location: Auditorium of National and Kapodistrian University of Athens

    First day of this year’s global conference on the diagnosis and treatment of hair loss was held at the auditorium of National and Kapodistrian University of Athens and it was attended by numerous DHI members from all around the globe, as well as world known scientists like Ron Shapiro MD (FISHRS, USA) and Francisco Jimenez MD (Dermatologist, FISHRS, Spain).

    During the first day, the attendees had the opportunity to watch for the very first time the official presentation of the new DHI research projects in collaboration with the Medical School of the National University of Athens.

    The conference started with an emotional 4.5 minutes long video dedicated to the 45 years of DHI Global Medical Group and first to speak was the Group’s founder and president, Kontantinos Giotis who talked about DHI’s Vission of changing the world of hair restoration from inside out, through education, research and innovative treatments.

    “The Evolution of Surgical Hair Restoration” was the subject of George Gounaris MD (Plastic Surgeon, DHI Medical Director) presentation and it was followed by George Liossis MD (Dermatologist, DHI Medical Director) who analyzed the unique DHI Diagnostic System of Alopecia (DSA).

    Vassia Birou MD (DHI Global Medical Director) and Georgia Ligda MD (Plastic Surgeon , PhD, DHI Medical Director) presented the DHI Total Care System and some Unique Patients Cases where DHI Medical Team managed to transplant a total of 10.034 hair on a patient with shaved head and 5.512 hair on an unshaven patient.

    Professor of Psychiatry in the University of Athens, Charalambos Papageorgiou, presented the new research project “Study of diseases of the hair and psychopathology. The psychological axis. Detection and intervention” which is aimed at in-depth study and exploration of psychiatric dimensions of hair diseases, in order to determine the necessity of combinatorial access of these conditions and any underlying psychopathology. Professor Papageorgiou said that all data results from this study will contribute to identify specific psychopathological problems such as anxiety and depression and all the patients will be informed in order to be given specific therapeutic proposals, if needed. This was followed by questions and discussion with the attendees.

    Professor Vassilis Gorgoulis Team and George Liossis MD (Dermatologist, DHI Medical Director) presented Hair Follicle Stem Cell Research, entitled “Study of human hair follicle stem cell niche: highlighting new perspectives on human hair restoration” which aims to study the characteristics of stem cells of hair follicles in humans and their immunological microenvironment in order to clarify their role in the cycle of renewal of hair follicles, the regeneration of hair and therefore, the effectiveness of follicular transplantation for the restoration of hair growth. The research project started five months ago and soon, there will be official announcements on the first results!

    Ron Shapiro MD (FISHRS) presented a truly interesting and educational study on the Creation of a Natural Hairline with Basic Principles, Guidelines and Techniques. Ron Shapiros presentation was followed by lot of questions questions and discussion with the doctors who attended AMM 2016.

    Francisco Jimenez MD (Dermatologist, FISHRS) presented the New Anatomical View of the Follicular Unit which has been his latest research project for the last year.

    The first day of the global conference on the diagnosis and treatment of hair loss at the University of Athens ended with the draw for the winner of the Free DHI Giga Hair Transplant up to 10.000 hairs and with Round Table Discussion.

    Saturday night, the attendees from all over the world had Greek dinner at the Aegeon Hotel at  Sounio.

    The second day of Academy Masters Meeting was held at the Aegeon Hotel in Sounio with the fantastic sandy beach and under the imposing temple of Poseidon.     Today was the annual London Hair Restoration Academy Meeting at Athens University where we presented our latest innovations. After all, this meeting is about education and innovation. To be honest, all the well known doctors who do FUE have started their carreer from DHI. Personally, i can name at least ten doctors who started from DHI. It is like a training ground for a lot of good doctors in hair restoration. They are doing it for years, there is so much experience and still try to make things better. Ten years ago, FUE was like a baby child and still today, is not that easy for all doctors. DHI is doing it from the birth of it.  I am very curious to see what the research projects will show.

    Two pioneering scientific researches which are revolutionizing the world of scalp treatments

    In the auditorium of the University of Athens, attendees got a first look at DHI’s new research project in collaboration with the Medical School of the National University of Athens, led by Professor Vassilis Gorgoulis.

    The collaboration studies the characteristics of hair follicle stem cells in humans and their immunological microenvironment in order to clarify their role in the cycle of renewal of hair follicles, the regeneration of hair and, therefore, the effectiveness of follicular transplantation for the restoration of hair growth. Professor Gorgoulis’ team stated that from the samples that have already been tested, they have completed the identification of regenerative cells for human hair follicles and have also advanced toward determining the overexpression of Cdc6, a key protein in hair health.

    The research project, begun five months ago, is entitled «Study of human hair follicle stem cell niche: highlighting new perspectives on human hair restoration.» The final results are expected in the next few months.

    More exciting research was presented during the Conference, including “Study of diseases of the hair and psychopathology. The psychological axis. Detection and intervention,” a collaboration between DHI Global Medical Group and the Psychiatric Clinic of the University of Athens-Aiginition Hospital. The research is an in-depth study and exploration of psychiatric dimensions of hair diseases, in order to determine the interrelation of these conditions and any underlying psychopathology.

    Referring to the pioneering research project, Professor of Psychiatry in the University of Athens, Charalambos Papageorgiou, stated that part of the study will be completed soon and scientists expect significant results.

    The research is carried out with the voluntary participation of DHI patients by completing questionnaires. All data resulting from this study will contribute to the assessment of psychopathologies present in requests for hairs disease treatment, to identify general psychopathology problems (anxiety, depression) impacted by hairloss, to identify specific problems (for example, body dysmorphic diorders) and of course, to advise patients and propose suitable medical interventions. The Academy Master’s Meeting is organized annually by the London Hair Restoration Training Academy, a division of the DHI Global Medical Group.

    • Taking hair from someone’s head and gluing it on your head’s affected area.
    • Taking hair from some other parts of your body and placing them on your head.
    • Cutting hair from your head and placing them back where needed.
    In reality, hair transplant is nothing of the aforementioned fantasies. It entails plucking of hairs from the head where they are really thick and inserting them where you need them the most. All hair transplants techniques are not same Yes. There are three types of hair transplant techniques: DHI, FUE, and FUT. All three are completely different from each other. DHI is the most advanced technique that gives you a natural hairline while FUE has the tendency to turn one into a zombie at times. FUT has become extinct.
